I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. I further acknowledge that the property & associated organizations have put in place preventative measures to reduce the spread of the Coronavirus/COVID-19. According to the CDC, it is still possible to spread and contract COVID-19 even with masks and social distancing. Tennessee has joined many other states in broadly limiting liability lawsuits related to Covid-19 exposure.

Entry and use of facilities and attendance at this event is voluntary and guests are subject to strict enforcement of the rules. Failure to follow posted rules may result in the guest being asked to leave if guest fails to comply with the safety guidelines. Warnings are a courtesy and a guest may be asked to leave after a single violation of the safety protocol such as wearing a mast appropriately and maintaining six feet of space between yourself and other patrons outside of your pods. Your ticket, wrist band, or lanyard pass is a revocable license and not is not transferable. PLEASE COMPLY WITH THE RULES! (We don’t make the rules, the Government does that. Please do not assault or yell at security for doing their job).

I acknowledge Bigfoot Electro, the property owner(s) & associated organizations, merchants, artists, and other hosts cannot guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, event staff, and other event clients and their families.

I attest that:

1. I am not experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell.

2. I have not traveled internationally within the last 14 days.

3. I have not traveled to a highly impacted area within the United States of America in the last 14 days.

4. I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.

5. I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non-contagious by state or local public health authorities.

6. I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.

I hereby release and agree to hold Bigfoot Electro, the property owner(s), artists, event staff, volunteers & associated organizations harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the salon, or that may otherwise arise in any way in connection with any services received from the property & associated organizations. This liability waiver and release extends to the promoter together with all owners, partners, and employees.

I voluntarily seek services provided by the property & associated organizations and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my appointment. I understand that this release discharges the property, its owners & associated organizations from any liability or claim that I, my heirs, or any personal representatives may have against the salon with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from the property & associated organizations.
